The "he" there is referring to his father. He didn't hear his grandma repeat those words. The words came from his father's mouth, so he's practically telling us what his father said about his grandma. I guess he loved the man very much at that age. The song was sang with so much passion and pain. Many people had a hard time figuring what he was talking about until he was interviewed.
