- Glass etcher
- Candle
- Matches or a lighter
- A cold water source
- Sandpaper
- Bandages just in case mistakes are made

I have also seen people tie a string around the neck of a bottle and burn it to create a small hot region vulnerable to cracking when cooled, so the glass etcher may not be necessary. It's probably a better way to get a reliable break point, though.
