Pseudothrombocytopenia and why it occurs? And how to know about it?
Hello everyone, how are you all? Hope you all are doing well and i too. Today we are going to learn about an important laboratory finding and that we should know and should be careful about that. We can discuss about it through a case

# Case
 This is a case of 17 years male patient cane to opd and doctor advised complete blood count. The blood sample in EDTA tube was run in coulter counter machine
And the complete blood count values we got and are given below. You can see Hemoglobin, platelets and white blood cell counts decreased in number. Platelets are 65000 which is low in number. Because normal platelet count is 1.5 lakh to 4.5 lakh per cumm. This normal range we already know and have seen in the previous post.

Now the questions arise are is it true? That platelet count is low? As we got in coulter counter machine. Does this machine gives correct platelet count? how to confirm this?

Here comes the role of peripheral smear. As platelet counts are low, peripheral smear was made. When i have seen peripheral smear on high power(1000X magnification) to do manual platelet count i got to see these things.

Platelet clumps are seen. So normally one platelet on high power denotes or equal to 10000 platelets. So when we do count manually we have to count in 10 High power fields that how many platelets we got and we have to get average of it. Here you can in the images platelet clumps are there. Each platelet clumps having 4-5 platelets. So this small platelet clumps are counted in single platelet in coulter counter which give low platelet count. Thats why it is called pseudothrombocytopenia. As i counted i got manual platelet counts near to 1.5 lakh which normal. So the coulter counter machine given 65,000 platelet count is not true and we should not be afraid of it as we got correct platelet count on peripheral smear.

So my advise is don't always rely on coulter counter machine values. Plz confirm with peripheral smear and do manual platelet counts. It gives true values. Hope you understood its importance.
